Probe by saying 'any other symptoms' and keep asking for more signs or symptoms until the mother/caretaker cannot recall any additional symptoms. Circle all symptoms mentioned, but do not prompt with any suggestions. . If you are not sure whether a certain sign as reported by the respondent fits in one of the above categories, write it down in full and check with your supervisor later. If the respondent mentions a symptom not listed here, write the description of the symptom in the space provided for 'Other' and circle 'X' ('Y', and 'Z', for additional symptoms not listed).
